Notice of Infringement – Claim
- A signature from the copyright owner or an authorized representative;
- Identification of the copyrighted material allegedly infringed;
- Details of the infringing material with enough information for us to locate it;
- Contact information of the complaining party including name, address, email, phone, and fax number;
- A statement of good faith belief that the material is being used without authorization;
- A statement of accuracy and authorization from the complaining party.

Counter Notification – Restoration of Material
If your material has been removed due to a copyright claim, you can send us a counter notification meeting specific requirements outlined in 17 USC Section 512(g)(3) to have it restored.
- Your signature;
- Description of removed material and its original location;
- A statement under penalty of perjury regarding the removal of material;
- Your name, address, phone number, consent to jurisdiction, and acceptance of service of process.
